SSA-3369
Describes the jobs you held in the years before you stopped working — what you actually did each day, how much you lifted and carried, how long you stood, walked, or sat, the tools you used, and whether you supervised anyone.
SSA-820
Reports work you did for your own business — the type of business, your ownership share, hours, income, expenses, duties, and any help or special arrangements you received.
SSA-821
Reports work you did for an employer — dates, wages, hours, subsidies, special conditions, work expenses related to your condition, sick or vacation pay, and unsuccessful work attempts.
SSA-3373
Describes an ordinary day — personal care, meals, chores, transportation, shopping, money, hobbies, social activity, physical abilities, concentration, instructions, stress, devices, medicines, and side effects.
SSA-454
Updates your file when the agency reviews whether benefits should continue — conditions, treatment, doctors, medicines, tests, hospital stays, schooling, work, rehabilitation, and changes in daily functioning.
SSA-561
Formally asks the agency to look at a decision again. You identify the decision, explain why you disagree, name your representative if applicable, and list supporting documents.
